Navigating the healthcare net zero countdown
By their nature, healthcare workers focus on sustaining our lives. Now, the institution that surrounds them is committed to becoming environmentally sustainable, as well; the NHS has committed to becoming the world's first net zero national health system.
In its aim to achieve this status, it has set two targets:
- Net zero by 2040: For all emissions controlled directly by the NHS, such as energy use and anaesthetic gases
- Net zero by 2045: For all emissions within the supply chain for the emissions it controls directly to be net zero by 2040
Yet one important date sits much closer. The NHS must reach an 80% reduction in its direct emissions, measured against a 1990 baseline, between 2028 and 2032. That window is approaching rapidly.
Progress is already being made, and the NHS' current pipeline features a new generation of net-zero designs, with integrated air-source heat pumps, extensive solar arrays, and high-thermal-efficiency insulation as regular specifications.
As ambition becomes a deadline, then, net zero and sustainability in NHS estates has begun to take a more solid shape. As this direction of travel becomes clearer, achieving net zero in healthcare construction is set to become one of the defining delivery challenges of the decade.
Upgrading infrastructure without disrupting patients
Achieving sustainability in healthcare buildings is difficult. Care cannot stop while the estate changes around it – you cannot shut down an operating theatre, for example, to install air-source heat pumps. Hospitals run 24 hours a day, and clinical continuity is non-negotiable.
Strikingly, the NHS accounts for around 40% of UK public sector emissions. Direct emissions are down just 14% since 2019/20 – only just on track, per the King's Fund. The early wins were clinical, such as clinical waste treatment and the near-complete phasing out of desflurane gas. NHS infrastructure must now support net zero goals for healthcare construction, as well as operational performance. That means heat, ventilation, power and fabric upgrades across live sites.
Funding is flowing, although it is time-limited. The latest Public Sector Decarbonisation Scheme (PSDS) round gave NHS bodies £408.1 million – half the UK's public decarbonisation budget – to spend by March 2028. Each scheme must also clear NHS business case approvals and the Net Zero Building Standard.

Rigorous stakeholder and clinical alignment
As with many complex construction projects, healthcare decarbonisation programme succeeds or fails on the strength of its alignment. A full range of stakeholders, including estates teams, clinical leads, infection prevention and control and trust boards must feed into a shared build schedule from the earliest stages.
Decant plans, in which patients and staff are relocated, as well as phased handovers and the scope of out-of-hours working, are crucial project decisions that must be made. The appropriateness of these plans determine whether patient care is disrupted. Robust project programming
Decarbonising a live hospital estate is a phased sequence, more so than a single project. Heat decarbonisation depends on a site’s electrical capacity. Fabric upgrades, in turn, will change the heat load. Each work package can alter the requirements of the next. Sequencing these interdependencies – and running tightly scheduled procurement alongside them – is what separates programmes that hit funding deadlines, like the March 2028 PSDS cut-off, from programmes that are compelled to hand money back. Compliance and business case governance
NHS capital projects are faced with stringent compliance parameters. There are business case approval gateways to navigate, and projects must meet the Net Zero Building Standard and carbon reporting obligations. Suppliers similarly face a net zero deadline. From 1 April 2027, all suppliers bidding for NHS work must, regardless of the value of the contract, publicly report their targets and emissions and publish a Carbon Reduction Plan covering all their global Scope 1, 2 and 3 emissions. From 2030, demonstrable net zero progress becomes a condition of qualifying for NHS contracts at all. Net zero readiness now matters to contractors as much as to trusts; for the trusts themselves, a supplier's compliance is one more procurement risk to screen at tender stage.
